Here is the questionnaire! Now what you are going to do is copy and paste this, answer the questions, then email your answers to Kristy at kristy.gibson@comcast.net or facebook. Try your best to answer all the questions, however if you absolutely do not have an answer to a question then just leave it blank. Just to let you know, no one will be singled out for any of their responses.
